Good question!

That guy has a lot of time on his hands. Looks good though. Hmm, his source code could be rewritten to add whatever tag we want.

You can type something, but then have a line going through it like it's being crossed out and you didn't want it there. It's nifty!

Its actually possible to add your own buttons.
What that FAQ answers is that you can't through phpbb (i.e. through add-ons or through the admin controls). if you really want a button, you can sift through the php source code and add it. Recompile, modulate and upload. I did that once for a friend of mine.
It is, however, obinoxiously painful to do.
